These three case studies are:

TeleClinical Care (TCC) – A digital health platform integrating telehealth and remote monitoring to support thousands of patients managing chronic disease safely from home.

Diabetes Case Conferencing – A GP–specialist collaboration model that improves diabetes management, reduces unnecessary referrals, and strengthens primary care capability.

Precision Care Clinic – A multidisciplinary genomics-driven model transforming cancer care through personalised treatment and hereditary risk assessment.

The case studies show how SPHERE’s partnerships, seed funding, and research translation expertise are turning ideas into scalable, patient-centred solutions that improve outcomes and equity in healthcare.
