This large-scale RCT is designed to determine the efficacy of Home Medicine Reviews conducted by pharmacists for deprescribing drugs that can cause falls and subsequent fractures in people with osteoporosis. Fractures in the elderly are common and may be precipitated by the use of drugs that increase the risk of falls. Medication management may reduce the risks of harm due to falls in this group. Given the aging of the population, interventions to reduce the burden of falls and fractures in the elderly is important for both the health of the elderly and reducing the burden of falls and fractures on the hospital system.
