Seed Grant – Care and management of existing cardiovascular disease for older people using community-based and residential aged care services

There is increasing recognition of the importance of palliative care for patients with life-limiting cardiovascular conditions including heart failure (HF), associated with high morbidity and mortality with associated high use of health services. The study found outpatient palliative care was associated with lower emergency department visits (and associated intensive care admissions and invasive ventilation procedures) and lower in hospital palliative care use, as well as increased GP visits. A collaboration with the Bankstown outreach team to translate findings to reach target audiences, as well as extending the work to other cardiovascular conditions and extending to other regions.
